PARTICIPATION AGREEMENT

INTRODUCTION

The Department of Informatics at Fort Hays State University supports the practice of protection for human subjects participating in research. You are being asked to participate in a research study. It is your choice whether or not to participate. The following information is provided for you to decide whether you wish to participate in the present study. You may refuse to agree to this form and not participate in this study. You should be aware that even if you agree to participate, you are free to withdraw at any time. If you do withdraw from this study, it will not affect your relationship with this unit, the services it may provide to you, or Fort Hays State University.

PURPOSE OF THE STUDY

The purpose of this study is to evaluate a series of website designs for usability. Results of this study may help improve future website designs.

PROCEDURES

As a participant in this study, you will be asked to find and remember three different items of information on a webpage. Your memory of each item will be tested after viewing each page. This process will be repeated for four different webpage designs. Finally you will be asked to complete a short survey comparing your experiences with each of the four webpage designs.

If you decide to participate in this research study, your consent will be implied by your participation. At any time you may choose to withdraw with absolutely no consequences. After you have read all of the information on this page the study may begin. The length of time of your participation in this study is approximately 15 minutes.

RISKS

There are no anticipated risks associated with this study.

BENEFITS

The only anticipated benefit associated with this study is the knowledge that you will be helping in our understanding of website usage and design.

PAYMENT TO PARTICIPANTS

No compensation is offered for participation in this study.

PARTICIPANT CONFIDENTIALITY

No records of any kind will be kept linking you to this study. Your participation in this study is completely confidential. We only ask for basic demographic information, which helps in understanding our results
